Factors Favoring Penile Fractures in Ivory Coast in 2022

Abstract
Obj ective : To report the factors promoting penile fracture. Patie nts and Method : This was a retrospective cohort study consisting of 27 patients received from 2021 to 2022 in urology at the University Hospital Center (UHC) of Cocody for penile fracture. The medical history made it possible to note the circumstances of the occurrence of the fracture, in particular the age of the patient, the sexual position during the sexual act, the consumption of products to stimulate erection. Results : The mean age of the patients was 35 years with age borderlines of 25 and 52 years. The circumstances of occurrence were dominated by the misstep during coitus in 26 out of 27 patients. The doggy style position was found in 18 patients and the cowgirl position in 8 patients. Among the patients, 17 took alcohol before the sexual act and 20 took traditional medicines (aphrodisiac) to stimulate their erections. Among the patients, 12 were living with a partner and the other 15 were not. One case of recurrence was noted. Early surgical treatment was performed in all patients. The circumferential approach was performed in all patients. The postoperative hospitalization period was 2 days. After surgical treatment, 2 patients had moderate pain during erections, there was no erectile dysfunction or penile curving. Relative risk factors were the cowgirl position, alcohol, and taking aphrodisiac drugs. Conclusion: Penile fracture is a rare traumatic emergency in urology occurring most often in a young population in full sexual activity. These young people are mostly unmarried, adopt more doggy style and cowgirl positions and perform the sexual act vigorously under the influence of alcohol and traditional erection-stimulating drugs.
